There are currently 3 different options for becoming a patient at FMHC.

Membership Model
A 6-month program working 1:1 with Dr. Walker as well as regular check-ins with a health coach.

Functional Foundations Program
Our Functional Foundations Program is a two-month Health Coaching Program that can be started before working with Dr. Walker.

The Gut and Skin Solution
A comprehensive 6-month package with health coaching, functional testing and working with Dr. Walker.

Insurance 
We do not bill insurance. However, you may consider submitting your bill to insurance for reimbursement. We can provide you with a receipt (superbill) if you would like to file a claim with your health insurance plan. Our services may or may not be covered by your insurance. Unfortunately, at this time Medicare patients cannot submit their bills.

Payment
Payment is due at the time of your service.

Cancellation Policy
Services are by appointment only. Because your appointment time is reserved only for you, it is important that you do not miss any appointments. There is a 24-hour cancellation policy; please cancel your appointment 24 hours in advance to avoid being charged an extra fee. If you do not show up for your scheduled appointment, you will be charged a $100 fee.
